The Early Childhood Wellness Plan (ECWP) 2016-2020
The Preventive Health Collaborative’s (PHC) Early Childhood Wellness Plan (ECWP) 2016-2020 provides PHC partners, stakeholders, advocates, & decision makers an easy way to track the health and wellness of children ages birth to five in Maricopa County.
The Early Childhood Wellness Plan 2016-2020 performs three functions:
- Provides a shared measurement system. The ECWP serves as a clearinghouse of indicators that our partners have identified as priority areas to track as we measure our impact.
- Provides direction. The ECWP provides targets for 2020. As the PHC keeps track of these indicators throughout the next four years, we will be able to assess which indicators are improving and which ones are not.
- Inform our partners about nationally-recognized best practices. The ECWP provides information on evidence-based and promising programs that our partners can implement to collectively improve our ECWP health indicators.
We will formally release the plan in February after our Partner meeting. The plan is a living document and we encourage your input. Email us back and let us know what you think!

PHC Action Learning Teams Update
Early, Periodical, Screening, Diagnosis, Treatment (EPSDT) ALT
Aim: Collaborate to increase well-child visits.

Safe Reduction ALT
Aim: Reduce DCS reports and removals, increase prevention programs servicing the area, increase family involvement and build a more robust community infrastructure.

- Finalized strategic planning
- Developed a community resource guide
- Performing direct outreach to high-incidence zip codes

Early Childhood Nutrition ALT
Aim: Families with young children (prenatal - 5) in Maricopa County provide nutritious food and develop life-long healthy eating habits.
Team Leads: First Things First, Pinnacle Prevention
Progress:
- Strategic planning underway
- Quarters 1,2 and 3 planing finalized
- Identified geographical areas to focus in: Alhambra, West Mesa.

Issac Community Health Hub
Aim: Create a Community Health Hub in the Issac School District to increase access to care.
Team Leads: Isaac School District, United Health Care Community Plan of Arizona, United Way - Valley of the Sun
Progress
- Secured a house to be developed into a community health hub
- Secured City of Phoenix IDA funds
- Strong representation from community leaders and residents
- Received beautification funds from Valley of the Sun United Way
